APPLY FOR A GRANT

The applicant must submit:

1. a complete application, located here;

2. proof of income (forms accepted are last paycheck stub, unemployment benefits, or disability benefits); 

3. a copy of the written cost estimate for requested assistance from the veterinarian, including name, email address and mailing address of veterinarian (contact us if you do not yet have a cost estimate); and

4. at least one picture of your pet in need of medical care (to be used on our website).
